Front Line Supervisor


Front Line Supervisor reports to Treatment Coordinator


  • B.S. or B.A. degree in Human Services or a related field preferred; high school diploma required and a minimum of one years’ working experience in the human service field.
  • Ability to operate and transport a member in a motor vehicle, have a current state driver’s license and satisfactory certificate of automobile liability insurance, and provide satisfactory evidence of a safe driving record (Pass DCI).
  • Ability to lift up to 50 pounds
  • Able to pass criminal background check
  • Able to pass abuse registry check
  • Previous experience with persons with disabilities is preferred but not required.
  • One year or more of supervisory experience is preferred.


  • To become certified in Child and Dependent Adult Abuse Reporting, all Agency Orientation modules, and all other required classes correlated with the DSP position.
  • Good communication skills and the ability to work well with others as a team.
  • Is computer and smart-device literate.
  • Able to use good judgement in a crisis and on a daily basis.
  • Carry a cell phone at all times and respond in a timely manner.
  • To become knowledgeable of the HCBS State Outcomes



Ÿ     Completes required direct care hours per week.

  • Assures that staff is trained accordingly on member goals and support needs; assures staff documents services rendered correctly; assures that Members’ rights are honored as listed in the ICP.
  • Completes all areas of the CSO training thoroughly with staff members prior to staff working with the Member; completes in-home trainings with staff as scheduled; assures staff trainings are documented correctly.
  • Maintains a positive rapport and communicates in a professional manner with Members served, their family members/ guardians, co-workers, case managers, staff members and others in the community.
  • Assures that quality systems are in place for Members: i.e. budgeting and money management; planning and running of doctor’s appointments; chore/activity schedules; health and safety protocols; monitoring and setting up medications; monitoring health concerns



  • Acts as a part of the Members’ Interdisciplinary Team; attends, provides input and/or provides updates for Member assessments, annual meetings, quarterly meetings and quarterly reports, as required.
  • Completes all Member paperwork in a timely manner, e.g. Medication Logs, meeting notes, weekly site checks, site actuals, waiver documentation.
  • Adheres to designated personal work schedules; attends permanently schedules shifts, attends work on time, does not leave shift early or change shift times without notification of the Treatment Coordinator.
  • Reviews, completes and reports changes to Member and Staff schedules by identified due dates. Reviews scheduling software and Time-Off Request lists to fill open shifts. Completes other needed timesheet review and payroll processes by identified duties by due dates.
  • Turns in permanent schedule changes, temporary schedule changes, staff training and CSO timeframes by designated due dates to the Treatment Coordinator and/or HR representative to be implemented into scheduling software for accurate schedules.
  • Completes projected schedules, follows projected schedules, and updates projected schedules. Turns in projected schedule weekly to Coordinator by due dates.
  • Reports supervisor hours worked correctly and by due dates.
  • Assists with staff re-trainings; adheres to outlined policies in handbook; communicates problematic employee behavior to the Treatment Coordinator and/or Human Resources; turns in re-trainings, or other documentation used for intervention purposes in a timely manner as requested by Treatment Coordinator and/or HR.
  • Communicates pertinent information in a timely manner to the Treatment Coordinator, other agencies such as Case Management, workshop, school, and nursing provider, staff, family member / guardians and Members.
  • Maintains a professional relationship and confidentiality with staff members and Treatment Coordinators by not discussing personnel issues with non-relevant persons.
  • Assures Members have access benefit resources (i.e. rental assistance, SSI, HUD, heating assistance, rent reimbursement and food stamps.)
  • Assures Members live in a safe environment with adequate furnishings. Completes required weekly Home Checks and Individual Home Safety Assessments and submits to the Treatment Coordinator by established due dates.
  • Assures Members have productive use of their free time, including social, recreation and leisure activities.
  • Turns in Member paperwork needed for office file maintenance to the Coordinator (i.e. medical forms, financial paperwork etc.)
  • Attends mandatory management meetings required for position; reviews management reminders and/or meeting minutes as required for position.
  • Follows up with Quality Assurance Department on quality items such as incident reports, home checks, office file reviews and other requests made from the Quality department.
  • Follows up with other departments (i.e. HR, Nursing, Financial, Training) in a timely manner when a request is made.
  • Develops member Lifestyle Plans, service documentation and completes the annual LP review, as assigned by Treatment and/or Quality Coordinator.
  • Completes quality assurance duties, including member office file reviews, member home and safety checks, employee personnel file reviews and service documentation reviews, or other tasks as assigned by the Quality Coordinator.
  • Completes data collection from service documentation and incident reports for members and/or Quality Coordinator.
  • Completes staff timesheet collection/review processes, reconciling processes, and payroll/billing processes, as assigned by Treatment, Financial and/or HR Coordinator.
  • Completes administrative scheduling duties as assigned by Treatment and/or HR Coordinator.
  • Completes additional administration or managerial duties, as assigned.


  • Must be able to work flexible hours; must be able to work evenings, overnights, weekends, and holidays as needed.
  • Supervisor will be on-call with a cell phone provided by NCI. Emergency phone calls must be returned within 15 minutes.




An employee with satisfactory job performance, employed  for more than one year, who experience circumstances  that temporarily affect or prevent the person from driving or operating a motor vehicle in performance of their job,  or experience a health or medical  condition that  temporarily affect the person’s ability or qualifications to drive or operate a motor vehicle required by their job, may request a waiver by the employer from driving or operating requirements in the employee’s  job description .
