Overview
We are seeking an experienced and motivated HCBS Treatment Coordinator to lead staff and ensure high-quality services for individuals receiving Home and Community-Based Services. This role focuses on compliance, staff development, and person-centered support.
Key Responsibilities
- Supervise and support Treatment Supervisors and direct care staff
- Ensure compliance with HCBS state and federal regulations
- Oversee service delivery and documentation (ICP/OAP, progress notes, med logs, etc.)
- Conduct quality assurance checks in homes and office files
- Coordinate care including medical appointments, medications, and daily living supports
- Attend required meetings (ICP, annuals, 3-month reviews)
- Maintain communication with families, case managers, and providers
- Assist with staff training, coaching, and performance management
Qualifications
- Bachelor’s degree in Human Services or related field preferred
- OR High school diploma with 5+ years of experience in human services
- 1+ year supervisory experience preferred
- Valid driver’s license and reliable transportation
- Ability to pass background and abuse registry checks
- Ability to lift up to 50 lbs
