HCBS Treatment Coordinator/Direct Support Leadership

Overview

We are seeking an experienced and motivated HCBS Treatment Coordinator to lead staff and ensure high-quality services for individuals receiving Home and Community-Based Services. This role focuses on compliance, staff development, and person-centered support.

Key Responsibilities

  • Supervise and support Treatment Supervisors and direct care staff
  • Ensure compliance with HCBS state and federal regulations
  • Oversee service delivery and documentation (ICP/OAP, progress notes, med logs, etc.)
  • Conduct quality assurance checks in homes and office files
  • Coordinate care including medical appointments, medications, and daily living supports
  • Attend required meetings (ICP, annuals, 3-month reviews)
  • Maintain communication with families, case managers, and providers
  • Assist with staff training, coaching, and performance management

Qualifications

  • Bachelor’s degree in Human Services or related field preferred
  • OR High school diploma with 5+ years of experience in human services
  • 1+ year supervisory experience preferred
  • Valid driver’s license and reliable transportation
  • Ability to pass background and abuse registry checks
  • Ability to lift up to 50 lbs
